Aligning Clinical Documentation with Code Selection

Every CPT, HCPCS, and ICD-10-CM code submitted on a claim tells a clinical story. When documentation clearly supports the complexity of the encounter and the medical necessity of each service, claims adjudicate smoothly and withstand payer review.

Evaluation and Management (E/M): MDM vs. Total Time

For office and other outpatient E/M visits (99202–99215), clinicians select the code level based on either the level of Medical Decision Making (MDM) or the total time spent on the date of the encounter.

  • Number and complexity of problems addressed during the encounter.
  • Amount and/or complexity of data to be reviewed and analyzed (tests, external notes, independent historians).
  • Risk of complications and/or morbidity or mortality of patient management.

High-Scrutiny Modifiers to Validate Before Billing

Modifiers such as Modifier 25 (significant, separately identifiable E/M service on the same day as a procedure) and Modifier 59 / X-modifiers (distinct procedural service) receive frequent automated payer scrutiny. Validating that clinical notes document a distinct evaluation or separate anatomical site prevents automated bundling denials.
